Acute Suppurative Thyroiditis Associated with Piriform Sinus Fistula

Abstract

|
We report a case of acute suppurative thyroiditis associated with a left piriform fossa-thyroid fistula. We feel it is important to report this condition because it may not be as rare as is though and could easily go undetected unless a barium
swallowing study is performed.
A 12 year old male patients had a painful swelling of the left anterior neck and fever was seen at his first episoide of the disease. A barium meal revealed a fistula originating from the apex of the left piriform sinus. The fistula, a remnatn of
the
fourth pharyngeal pouch, thus seems to be a common route o infection in aucte ssuppurative thryoiditis, following bacterial infection to begin in the perithyroidal space and spread to the thyroid gland. Complete extripation of the fistula is
require for
a permenant cure.
